Transitional Care at Tuscan Gardens

Transitional Care at Tuscan Gardens is a specialized level of senior living designed for seniors whose needs fall between Assisted Living and Memory Care. Often described as a bridge program, this unique level of care supports residents who need more hands-on assistance, structure, and daily oversight than Assisted Living typically provides, yet do not require a secured Memory Care environment.

Our program is ideal for seniors experiencing mild cognitive changes, increased physical or medical needs, or early difficulty managing daily routines such as medication adherence, personal care, or orientation throughout the day. Transitional Care is appropriate for individuals who are not at risk of wandering or elopement, but who benefit from closer supervision, gentle cues, and a higher level of personalized support.

Our Transitional Care neighborhood, The Village, is located on the second floor of our Memory Care community, but unlike Memory Care, it is not secured. Residents enjoy the benefit of enhanced staffing, experienced caregivers, and proximity to specialized resources, while maintaining the freedom to come and go throughout our beautiful community. This allows residents to participate fully in Assisted Living activities, dining, and social gatherings, preserving independence, choice, and connection.

Life in The Village balances additional care with meaningful engagement. Residents receive assistance with activities of daily living, medication management, and personal care, along with consistent guidance and encouragement throughout the day. At the same time, they remain an active part of the broader Tuscan Gardens community, enjoying social opportunities and shared experiences that promote confidence, familiarity, and emotional well-being.
